In space #1, in an unlocked cabinet, I observed Zep Disinfectant Wipes, and a can of aerosol spray disinfectant. The teacher locked the cabinet today during my visit.

The materials and equipment indoors and outdoors were not sufficient to provide a variety of play experiences to promote the children's emotional and social development, health and physical development, approaches to play and learning, language and communication development, and cognitive development. In space # 5 for two-and three-year-old children, I observed the dramatic play center to not have any pretend food or play dishes available for the children to use. You added some play foods and pretend dishes to the dramatic play area today during my visit.

All equipment and furnishings were not in good repair. The outdoor playgrounds were monitored today. I observed one plastic chair to be cracked and broken. The chair could pinch a child’s legs if they sat on it. On the toddler playground, I observed several rubber mats that were warped creating a tripping hazard. You removed the broken chair, and the warped rubber mats from the play area during my visit.

Parent's medication authorization did not include required information. In the classrooms a sampling of medications and the permission slips were monitored. I observed three diaper cream permission slips to be dated incorrectly and/or they did not contain the correct information. You removed the diaper creams from the classrooms today.

Incident reports were not completed each time a child was injured, it did not include all the information required in rule, it was not signed by the parent and/or it was not maintained in the child's file. A toddler teacher stated she doesn’t keep a copy of the centers the abbreviated injury form when it is completed. She said she sends the form home with the parents.
