Operator did not provide a physically safe and healthy indoor and outdoor environment that meets the developmental needs of the children in care. The bathroom door was open, and had unlocked cabinets and a basket that contained items labeled Keep Out of Reach of Children with additional warnings. Aloe wipes for pet's paws were observed in a bench also used for storage which were labeled Keep Out of Reach of Children with additional warnings. Ms. Tate placed this item into locked storage during the visit. An individual pump of hand sanitizer was observed less than five feet from the floor on a kitchen counter. This item was placed greater than five feet up during the visit. Three lighters, greater than five small bottles of hand sanitizer, a small canister of aerosol room spray, and two pairs of adult scissors were observed in an unlocked drawer accessible to children. Two cubby drawers located in the play room were observed to have plastic bags, and a children's book with foam pages was observed, each accessible to children, presenting a potential choking hazard for enrolled children. Ms. Tate placed each of these items into locked storage during the visit. The foam book and plastic bags were made inaccessible to children during the visit by Ms. Tate. A small hole was observed in the wall behind the door of the play room. To the left of the back door (facing the home), a small trash can was observed to have a cracked lid, sharp to the touch. To the far right of the back door (facing the home), one large trashcan was observed to have a cracked lid, sharp to the touch. A tiki torch was observed to have plastic flaking off the stem of the torch. Ms. Tate's husband removed each of these items from the outdoor play area during the visit. An outdoor table was observed to have a screw protruding from the base. Ms. Tate's husband screwed this screw into the base of the table during the visit. A small, metal table, the base of the larger outdoor table, and the top metal rails of the fencing were observed to be rusting. The round backing of a metal spigot located on the lower right side of the house (facing the home) was observed to have become separated from the paneling of the home, rusting, and sharp to the touch. At the corners of the playground where the metal fencing meets, and at all gate junctions where the gate doors either connect with each other, or with the fencing itself, greater than ten (10) bolts which secure the fencing were protruding inward from the gate, up to an inch beyond the nuts, presenting protrusion hazards. A large gas tank to the right side of the home (facing the home), used for heating, was observed to be rusting and to have paint flaking off the sides. | |||

Multiple cardboard boxes were located on the decking of the porch between the back door and the children's play area, posing a potential tripping hazard. Four open, empty aluminum cans were observed (either on the ground, or in a stack of empty flower pots) which were sharp to the touch. A screwdriver was observed laying on top of a planter box. An aerosol can of Krylon Color Master paint primer was observed sitting on a table with a built-in umbrella. A garden hoe sharp to the touch was observed laying on the ground in a garden box within the fenced area. A paint roller and paint tray with dried paint was observed on the grass near the deck within the fenced area. A long cord hung from the roof of the back porch down onto the porch. This could present a potential strangulation hazard. Four metal gardening rods were observed lying against the fence on the inside of the fenced-in area. A camping propane canister and two Sterno cannisters labeled Keep Out of Reach of Children with additional warnings were observed in unlocked storage on top of a cabinet off the kitchen. Two AA batteries were observed plugged into a battery charger without a protective cover in the kitchen, accessible to children. Ms. Tate relocated this battery charger during my visit. A long phone charging cord and a laptop charging cord were observed coiled on the living room floor. A bundled string of Christmas lights was observed within children's reach to the left of the front door of the home. A pair of adult scissors was observed on a table in the living room. The operator's purse was observed on the floor of the living room; the purse was laying open and was observed to have prescription medication inside. A pocketknife was observed laying in a decorative dish on a shelf in the living room. All items observed in the living room were at a height accessible to children. A 3-wick candle labeled Keep Out of Reach of Children with additional warnings. was observed in the living room on a shelf. Two outlets in the kitchen that were not in use did not have safety covers on the outlets. Ms. Tate placed safety covers over these outlets during the visit. | |||
