Two unlocked staff bathrooms on the 500 hall with open entry doors had one aerosol can of disinfectant spray and one aerosol can of air freshener in each bathroom.

One staff member in the NC PreK classroom did not have a signed and dated acknowledgement of review of the Shaken Baby Syndrome and Abusive Head Trauma Policy.

Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The fire inspection was completed on October 17, 2022. I was emailed a copy of the fire inspection today.

Medications including prescription and non-prescription items were not stored in a locked cabinet or other locked container. Emergency medication in room 509 was not stored at least five vertical feet from the floor. The medication was placed on a shelf above five feet today.

A current activity plan was not posted for each group of children for reference. In room #508, no lesson plan was posted for review. The teacher stated he keeps the lesson plans on his computer. The teacher posted the current lesson plan today during my visit.

All equipment and furnishings were not in good repair. One cabinet in the preschool classroom, used for classroom storage, was closed, and secured with a child safety lock. When I opened the cabinet door, I observed the hinge was broken causing the door to almost fall off.

All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. In room #508, in two unlocked cabinets, I observed a can of aerosol spray disinfectant, five containers of Lysol and Great Value Disinfectant wipes, and a spray cleaning product. On a teachers desk below five vertical feet from the floor, I observed a bottle of liquid hand sanitizer.

Prescribed medicine was not in original labeled container or accompanied by signed and dated written instructions from prescribing physician or health care professional. In the preschool classroom, I observed an inhaler that was not in its original container with the original prescription label. In room #508, a medication authorization form was signed and updated by the doctor, but the prescription label on the medication was not current.

On or before the first day of work, all staff, including the director and individuals who volunteer more than once per week did not provide results indicating that they were free of active TB and/or TB test or screening was older than 12 months. Two employees did not have a TB test completed prior to their first day of work.
