Subsequent tuberculosis screening is not required unless directed by a physician, physician's assistant, CRNP, the Department of Health or a local health department. Correction Required: An operator shall notify local traffic safety authorities annually in writing of the location of the facility and the program's use of pedestrian and vehicular routes around the day care facility.

Noncompliance Area: DURING THE RENEWAL INSPECTION ON 9/12/19, THERE WAS NO INSTRUCTIONS FOR CONTACTING THE APPROPRIATE REGIONAL DAY CARE OFFICE POSTED IN THE SAME LOCATION WITH THE APPROPRIATE REGULATIONS.

Correction Required: The facility's current certificate of compliance and a copy of the applicable regulations under which the facility is certified shall be posted in a conspicuous location used by parents, with instructions for contacting the appropriate regional day care office posted at the same location.

Noncompliance Area: DURING THE RENEWAL INSPECTION ON 9/12/19, THERE WAS BROKEN ELECTRONIC EQUIPMENT UNDER THE TABLE IN THE INDOOR PLAYSPACE CREATING A VISIBLE HAZARD.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

Noncompliance Area: DURING THE RENEWAL INSPECTION ON 9/12/19, THE STAIRWELL FROM THE UPSTAIRS PLAYSPACE DOES NOT HAVE A BARRIER TO PREVENT ACCESS FROM THE CHILDREN DURING LARGE MUSCLE DEVELOPMENT..

Correction Required: If a door or doorway opens or exits directly into a stairwell and if there is no landing beyond the door or doorway, the door shall be restricted from opening or shall be removed and a secure barrier to prevent access to the stairwell shall be erected.

Noncompliance Area: DURING THE INITIAL INSPECTION ON 4/24/19, THERE WAS 3 FOLDING TABLES, A PEICE OF GLASS COVERED WITH DIRT AND RANDOM ELECTRONIC EQUIPMENT IN THE UPSTAIRS PLAYSPACE THAT WAS ACCESSIBLE TO CHILDREN POSING A POTENTIAL HAZARD.

Correction Required: Furniture must be durable, safe, easily cleaned and appropriate for the child's size, age and special needs, PRIOR TO THE ISSUANCE OF THE CERTIFICATE OF COMPLIANCE.

Noncompliance Area: DURING THE INITIAL INSPECTION ON 4/24/19, THE PROVIDER HANDBOOK DID NOT HAVE LANGUAGE REGARDING THE FACILITY'S DISMISSAL POLICY.

Correction Required: The operator shall review with the parent, at the time of application, the facility's general daily schedule, hours for which care is provided, fees, responsibilities for meals, clothing, health policies, supervision policies, night care policies, dismissal policies, transportation and pick-up arrangements, PRIOR TO THE ISSUANCE OF THE CERTIFICATE OF COMPLIANCE.

Noncompliance Area: DURING THE INITIAL INSPECTION ON 4/24/19, A WRITTEN PLAN IDENTIFYING THE MEANS OF TRANSPORTING A CHILD TO EMERGENCY CARE AND STAFFING PROVISIONS IN THE EVENT OF AN EMERGENCY WAS NOT POSTED IN THE FIRST FLOOR OR SECOND FLOOR CHILDCARE SPACE.
