Child care administrators did not complete the ITS-SIDS training within 90 days of employment and every three years thereafter. After review of the staff and training worksheet it was noted that a floater that works in the infant room had not completed the required ITS-SIDS training.

Children were not adequately supervised at all times. The administrator reported on January 30th, 2023, three (3) staff members were outside on the playground with seventeen (17) children ages three-through-five years old. One (1) teacher was involved in a music activity with the children, another teacher was getting children to line up to do a head count and the third teacher was getting ready to leave for the day when one staff noticed that two (2) children were coming out from behind the climbing wall. After the administrator became aware of the situation and received more details, two children reportedly were inappropriately touching each other without the staff's knowledge. Staff were unaware the two (2) children were located behind a climbing wall located on the playground.

The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. There were three (3) staff members that did not completed the required training within ninety (90) days of employment. One (1) staff was hire on 9-10-2021 and did not complete training until 3-10-2022. One (1) staff was hired on 10-29-2021 and did not complete training until 4-13-2022. One (1) staff was hired on 10-11-2021 and did not complete training until 3-06-2022.
