Noncompliance Area: In review of children's files, child 1, enrolled date 09/14/19, failed to have a child health report contained in file.

Correction Required: The operator shall require the parent of an enrolled child, including a child, a foster child and a relative of an operator or a facility person, to provide an initial health report no later than 60 days following the first day of attendance at the facility.

Noncompliance Area: In review of staff files, staff 3 failed to have a health assessment contained in file.

Correction Required: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem. A facility person's record shall include a written report of initial and subsequent health assessments, including the results of initial and subsequent tuberculin skin tests, x-rays or other medical documentation necessary to confirm freedom from communicable tuberculosis.

Noncompliance Area: In review of staff files, staff 1, hired 11/18/19, staff 2, hired 11/18/19 and staff 5, hired 11/16/19, failed to have mandated reporter training contained in file. Staff 4, hired 12/17/19, failed to have NSOR clearance contained in file.

Correction Required: A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL.

Noncompliance Area: In review of staff files, staff 4 file failed to contain verification of 2 years of child care experience prior to employment at facility.

Correction Required: An assistant group supervisor shall have a high school diploma or a general educational development certificate and 2 years experience with children. A facility person's record shall include verification of child care experience, education and training prior to service at the facility.

Noncompliance Area: During renewal inspection on Monday, 01/27/2020, cert rep observed window in room 3 opened more than 6 inches.

Correction Required: Windows or doors above the ground floor that open directly to the outdoors and are accessible to children shall be constructed, modified or adapted to limit the opening to 6 or fewer inches.

Noncompliance Area: In review of staff files, staff person 1 failed to have verified 2 years of experience with children prior to employment at the facility. Only 1 year of experience working with children was contained in staff file.

Correction Required: A group supervisor shall have an associate's degree from an accredited college or university in early childhood education, child development, special education, elementary education or the human services field and 2 years of experience with children. A facility person's record shall include verification of child care experience, education and training prior to service at the facility.

Noncompliance Area: During renewal inspection on Friday, 01/25/19, cert rep observed trashcan located in boys bathroom failed to have a lid.

Correction Required: A toilet area, training chair area, diapering area and sink area shall be equipped with a clean, lidded waste receptacle.

Noncompliance Area: During compliant investigation on Friday, 10/26/18, cert rep observed infants and young toddlers in the infant room sleep during nap time in bouncy chairs.

Correction Required: Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the child's parent and the operator. The rest equipment shall be labeled for the use of a specific child and used only by the specified child.

Noncompliance Area: During compliant investigation on Friday, 10/26/18, cert rep observed preschool age children going to the bathroom alone and unsupervised.

Correction Required: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Children must be supervised at all times.

Noncompliance Area: While reviewing children's files, certification supervisor observed that the file for child #5 contained a child service report that was not current. The last child service report in file for child #5 was from 2016.

Correction Required: The services to be provided to the family and the child, including the Department's approved form to provide information to the family about the child's growth and development in the context of the services being provided. The operator shall complete and update the form and provide a copy to the family in accordance with the updates regarding emergency contact information in 3270.124(f) (relating to emergency contact information).

Noncompliance Area: While reviewing children's files, certification supervisor observed that the emergency contact forms for children #1-#4 did not contain addresses of individuals to whom children may be released.

Correction Required: Emergency contact information must include the name, address and telephone number of the individual designated by the parent to whom the child may be released.

Noncompliance Area: While reviewing children's files, certification supervisor observed that the emergency contact form for child #5 had not been signed and/or updated with a 6 month period.

Correction Required: The parent shall update in writing emergency contact information once in a 6-month period or as soon as there is a change in the informationA parent is required to review and update the emergency contact information and the financial agreement at least once in a 6-month period or as soon as there is a change in the information.

Noncompliance Area: While reviewing staff files, certification supervisor observed that the health assessment for staff persons #2 and #4 did not have answers to the questions about the staff's suitability to serve as a staff person and being free from communicable diseases.

Correction Required: An adult health assessment must include an examination for communicable diseases and the results of that examination. An adult health assessment must include the physician's or CRNP's assessment of the person's suitability to provide child care.

Noncompliance Area: While reviewing staff files, certification supervisor observed that the health assessments for staff persons #2 and #4 did not have answers to the questions about the staff's suitability to serve as a staff person and being free from communicable diseases.

Correction Required: A facility person's record shall include a written report of initial and subsequent health assessments, including the results of initial and subsequent tuberculin skin tests, x-rays or other medical documentation necessary to confirm freedom from communicable tuberculosis.

Noncompliance Area: While reviewing facility files, certification supervisor observed that the file for staff person #1 contained an FBI clearance for the Dept of Education which had a criminal history. Additionally, the file for staff person #3, who was hired at the facility on 9/25/17 and who sometimes works in the facility classrooms did not contain verification of completion of mandated reporter training.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). The operator is required to submit to the Children and Youth Agency a plan of supervision or alternate arrangement as required by the Child Protective Services Law. Facility Person #3 may not work in a child care position at the facility until mandated reporter training is completed. A facility person's record shall include verification of child care experience, education and training following the outset of service at the facility.
