Noncompliance Area: The doors of the play sink and refrigerator were not securely attached to the equipment creating pinch points. Exposed nails were accessible to children on the door of the play sink.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Noncompliance Area: The right hand door on the video cabinet was not secured to the cabinet creating pinch points. The cabinet sits on the floor and is accessible to children.

Correction Required: Furniture must be durable, safe, easily cleaned and appropriate for the child's size, age and special needs.

Noncompliance Area: The files for the following children did not contain documentation of any completed child services reports; child #1 enrolled since 8-21-19, child #2 enrolled since 9-4-18, and child #3 enrolled since 8-21-19. The reports were due prior to the regulation being suspended due to COVID-19.

Correction Required: An agreement shall specify the services to be provided to the family and the child, including the Department's approved form to provide information to the family about the child's growth and development in the context of the services being provided. The operator shall complete and update the form and provide a copy to the family in accordance with the updates regarding emergency contact information at 3280.124(f).

Noncompliance Area: The file for child #1 contained emergency contact information which did not include written consent signed by a parent for emergency medical care.

Correction Required: Emergency contact information must include the written consent signed by a parent for emergency medical care. A child's record must contain signed parental consent for emergency medical care for the child. Written consent is required prior to admission.

Noncompliance Area: The file for child #3 contained emergency contact information which did not include the health insurance policy number under which the child is covered.

Correction Required: Emergency contact information must include health insurance coverage and policy number for a child under a family policy or Medical Assistance benefits, if applicable.

Noncompliance Area: The files for children #3 and #5 contained emergency contact information which did not include the address of the individuals designated by the parent to whom the child may be released.

Correction Required: Emergency contact information must include the name, address and telephone number of the individual designated by the parent to whom the child may be released.

The file for child #3 contained emergency contact information and an agreement which have not been updated since 6-3-19. These updates were due prior to the regulation being suspended due to COVID-19.

Correction Required: The parent shall update in writing emergency contact information once in a 6-month period or as soon as there is a change in the informationA parent is required to review and update the emergency contact information and the financial agreement at least once in a 6-month period or as soon as there is a change in the information.

Noncompliance Area: The file for child #2 contained an initial health report conducted more than 60 days after the child's first day of attendance. The health report on file is now current.

Correction Required: The operator shall require the parent of an enrolled child, including a child, a foster child and a relative of an operator or a facility person, to provide an initial health report no later than 60 days following the first day of attendance at the facility.

Noncompliance Area: The file for child #3 contained a health report which did not include a review of the child's immunized status. The file did not contain any immunization records.

Correction Required: A health report shall include a review of the child's immunized status according to recommendations of the ACIP.The facility may not accept or retain an infant 2 months of age or older, a toddler or a preschool child at the facility for more than 60 days following the first day of attendance at the facility unless the parent provides written verification from a physician, physician's assistant, CRNP, the Department of Health or a local health department of the dates (month, day and year) the child was administered immunizations in accordance with the recommendations of the ACIP.

Noncompliance Area: The file for child #4, who is 18 months old, contained a immunization record which lacked documentation of having received the influenza vaccine in the past 12 months.

Correction Required: The facility shall require the parent to provide updated written verification from a physician, physician's assistant, CRNP, the Department of Health or a local health department of ongoing vaccines administered to an infant, toddler or preschool child in accordance with the schedule recommended by the ACIP.

Noncompliance Area: The file for child #1 did not contain signed parental consent for administration of minor first-aid procedures by facility staff.

Correction Required: A staff person shall obtain an annual minimum of 6 clock hours of child care training.A facility person's record shall include verification of child care experience, education and training following the outset of service at the facility.

Noncompliance Area: The file for staff #1 contained expired documentation of completing mandated reporter training. The training was last completed on 4/28/15. Based on documentation on file, staff #2, with a start date of 6/24/19, did not complete mandated reporter training within the required 90 days. The training was completed on 10/4/19.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person's record shall include verification of child care experience, education and training following the outset of service at the facility.

Noncompliance Area: The file for staff #1 contained clearances which were not updated within the required 60 month time period. The initial state police clearance was obtained on 5-11-15 and was updated on 6-22-20. The initial child abuse clearance was obtained on 5-5-15 and was updated on 6-25-20. All clearances on file for staff #1 are now current and valid.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL.

Noncompliance Area: Two electrical outlets in the front room, accessible to children 5 years old or younger, lacked protective covers.

Correction Required: Protective receptacle covers shall be placed in electrical outlets accessible to children 5 years of age or younger.
