Between 6:30am and 6:40am, nineteen (19) children, ranging in age from one (1) to seven (7) years of age were grouped together in space five (5), the classroom for four- to five-year-old children. Although there was adequate staff to ensure compliance with staff/child ratio, the maximum group size was required to be 12 due to the youngest child in attendance being one (1) year of age.

Children were not adequately supervised at all times. On Friday, March 15, 2024, a teaching staff member walked out of the classroom for two year old children, leaving the children alone and unsupervised. The classroom was without a teacher for approximately two minutes. On Monday, March 25, 2024, a child four years of age, left space 5 unsupervised. The child walked down the hall and entered space 4, the classroom for children one year of age. The teacher in space 5 did not know the child left the classroom.

Upon arrival, there were thirty-eight children present in space 5, ranging from one year of age to seven years of age. There was one teacher present in the room when I arrived, and an additional teacher arrived shortly after my arrival. Due to the classroom having a child one year of age present, the required staff/child ratio in this classroom would be 1/6. There were six children present in space 3, ranging in age from infant to two years of age, with one teacher present. This classroom is designated for infant-aged children; therefore, the required staff/child ratio in this classroom would be 1/5.

Children under one year of age were not kept separate from children two years and older. In space 3, there was a child two years of age present in the classroom for infant-age children. There were two infant-age children present, three children one year of age present, and one child two years of age present.

Staff did not wash their hands thoroughly before beginning work, before/after handling food, before bottle feeding or serving to other children, after toileting or handling body fluids, after diaper changing and after handling soiled items. In space 2, K. Roberson did not wash her hands after changing a pull-up. The same teacher assisted a child with washing hands; however, the steps in washing hands were not followed correctly.

A safe indoor and outdoor environment was not provided for the children. There were active fire ant mounds on the playground for school-age children. Paint was peeling off the walls in all classrooms.
