Disinfectant wipes labeled, "Keep out of Reach of Children" were accessible to children in an unlocked cabinet in kitchenette area.There were several large rips in the diaper table changing pad that was between the crib room and multi-purpose room making it not sanitizable.The baseboard was missing in the space along the wall outside of the crib room.In large main room where the children play there were five ceiling tiles with visible water stains and one was sagging. There was also half of a tile completely missing in that same area.In the main play area part of the wall where cots are stored was missing the boards and the drywall appeared to be exposed
