School-aged children were not adequately supervised. At 4:20pm a child enrolled in the group with kindergarten through second grade children was observed leaving the cafeteria area and walking towards the exit door where parents pick up. A group leader came out and asked the child where she was going. The child said she was leaving. The group leader escorted the child back into the cafeteria. There was a group leader standing at the bathrooms but was unaware the child had left the cafeteria and could not see or hear the child. | |||
