A safe indoor and outdoor environment was not provided for the children. In the outdoor area that the children use, the bottom portion of a mobile unit was damaged and loose screws, nails and the pipes underneath the unit were accessible by children. A garden hose was tangled up in the grassy area which was a tripping hazard. The wooden corner seats on the sandboxes were broken and had exposed nails. One of the wooden benches beside the sandboxes had splintered wood that could cut and/or scratch a child when sitting on it. The area around the large drain was hazardous as the dirt had washed away underneath the concrete and had created a tripping hazard with the uneven ground. Trash, a piece of roofing material and other items of debris were in the area accessible by the children. | |||
