Noncompliance Area: The file for Child #1, aged three-year-and-eight months, contained documentation of immunizations; however, only three of the four ACIP recommended PCV 13 doses were documented.

Correction Required: A health report shall include a review of the child's immunized status according to recommendations of the ACIP.

Noncompliance Area: A trach can located in the restroom between classrooms 22 and 23 was observed to be used for the disposal of pull-ups/diapers lacked a hands-free mechanism.

Correction Required: If disposable diapers are provided by a parent or by a facility, a soiled diaper shall be discarded by immediately placing the diaper into a plastic-lined, hands-free covered can.

Noncompliance Area: The following were observed in areas accessible to children in care: In classroom #1, a wooden, child-sized rocking chair was observed to have a sharply-pointed edge on a bracing piece on the underside of the seat of the chair. A wooden factory spool repurposed as an activity table was observed to have sharp, splintering areas around the circular table top. A wooden mailbox post was observed to have splintering edges. Two wooden benches were also observed to have sharp, splintering edges.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Noncompliance Area: During naptime, one child in classroom #1 and two children in classroom #2 were observed to be lying on rest equipment that did not have two feet of clearance on at least three sides.

Correction Required: At least 2 feet of space is required on three sides of a bed, cot, crib or other rest equipment while the equipment is in use.

Noncompliance Area: The files for Staff #1, #2, and #3, all of whom having been employed in child care positions for more than twelve months, did not contain documentation of the completion of fire safety training in the past year.

Correction Required: Staff persons shall participate, at least annually, in firesafety training conducted by a fire protection professional. Staff persons and volunteers shall receive training in maintenance of smoke detectors, the duties of facility persons during a fire drill and during a fire and the use of the facility's fire extinguishers, not including discharge of the fire suppressant agent.A facility person's record shall include verification of child care experience, education and training following the outset of service at the facility.

Noncompliance Area: The file for Staff #2 contained a signed/dated disclosure statement; however, the document was dated after Staff #2's start date in a child care position.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services).

Noncompliance Area: The file for Staff #3, who has been employed in a child care position for more than one year, did not contain a written evaluation.

Correction Required: A director is responsible for written evaluation of staff persons on a regular basis, a minimum of one evaluation every 12 months.

Noncompliance Area: In classroom #2, an approximately 3' high two-door wooden cabinet was observed to have a protruding screw and several flexible aluminum ties along the chain link fence enclosing the outside play space were observed to have sharp ends. Each of these areas of concern could cause laceration injuries, as they were observed in areas accessible to children in care.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

Noncompliance Area: In the outside play space, in an area accessible to children in care, an approximately 24" high x 48" long shelf as observed to have peeling paint in several areas.

Correction Required: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility.

Noncompliance Area: In child care space AS 1, in an area accessible to children in care, a plastic bin was observed to be cracked, which resulted in a sharp, jagged edge that could cause a puncture or laceration wound.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Noncompliance Area: In room AS 1, a child care space, a sharp-toothed staple remover was observed on a shelf that was accessible to children in care. The staple remover posed the potential to cause laceration or puncture.

Correction Required: Conditions at the facility may not pose a threat to the health or safety of the children.

Noncompliance Area: The file for Staff #1, whose start date in a child care position was 08/28/2018, contained a disclosure statement that was signed and dated on 09/25/2018.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services).

Noncompliance Area: The file for Staff #2, who is listed as an Assistant Group Supervisor, contained documentation of the completion of an Associate's Degree via a copy of a diploma. The diploma, however, did not indicate the major course of study. The file for Staff #2 did not include supporting transcripts; nor did Staff #2's file include documentation of child care experience.

Correction Required: A staff person or a substitute staff person shall meet one of the applicable staff qualifications for the position in which the person is performing.An assistant group supervisor shall have a high school diploma or a general educational development certificate and 30 credit hours from an accredited college or university in early childhood education, child development, special education, elementary education or the human services field.

Noncompliance Area: One toilet in each bathroom #1 and #2 were observed to have exposed, rusting bolts protruding from each of the sides of the toilets, which created the potential for puncture, laceration, and-or fecal contamination.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

Noncompliance Area: A plastic toy bin in Preschool room #2 was observed to have a broken, jagged edge where it appears a hinged lid had broken off.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.
