Correction Required: Stairways, hallways, exits from rooms, exits from the facility and other means of egress serving as an exit shall be unobstructed.

Noncompliance Area: AT THE TIME OF THE RENEWAL INSPECTION THE CERTIFICATION REPRESENTATIVE OBSERVED THAT CHILD #5 DID NOT HAVE RELEASE PERSONS LISTED ON THEIR AGREEMENT.

Correction Required: An agreement shall specify the persons designated by a parent to whom the child may be released.

Correction Required: A parent is required to review and update the emergency contact information and the financial agreement at least once in a 6-month period or as soon as there is a change in the information.

Noncompliance Area: AT THE TIME OF THE RENEWAL INSPECTION THE CERTIFICATION REPRESENTATIVE OBSERVED THAT STAFF #1 DID NOT HAVE VERIFICATION OF EDUCATION ON FILE.

Correction Required: A facility person's record shall include acceptable verification of experience, education or training is a transcript or a diploma or a letter signed by a representative of the experiential, educational or training entity.

Noncompliance Area: AT THE TIME OF THE RENEWAL INSOPECTION THE CERTIFICATION REPRESENTATIVE OBSERVED THAT STAFF #1 DID NOT HAVE A PA STATE POLICE CLEARANCE ON FILE.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). SAFETY PLAN: The operator is required to submit to the Children and Youth Agency a plan of supervision or alternate arrangement as required by the Child Protective Services Law. LACKING REQUIRED HIRING DOCUMENTS: Facility Person # -- may not work in a child care position at the facility. PROVISIONAL HIRE UNSUPERVISED: A provisional employee may not be permitted to work alone with children and must work within the vicinity of a permanent employee.

Noncompliance Area: During the inspection on 3/9/18 staff person #1 was observed not cleaning the changing surfaces after diapering and not washing their hands after diapering.

Correction Required: Diaper changing surfaces shall be cleaned after each use by wiping the surface with a sanitizing solution or by changing a pad or other surface covering. A facility person shall wash his hands before meals and snacks, and after toileting and after diapering a child.

Noncompliance Area: The facility lacked a sufficient amount of materials for dramatic role playing in the young toddler classroom.

Correction Required: Play equipment and materials appropriate to the developmental needs, individual interests and ages of the children shall be provided in sufficient amount and variety to preclude long waits for use. Play equipment and materials shall include materials for dramatic role playing. Obtain additional materials for dramatic role playing in a sufficient amount to preclude long waits. Submit written documentation of purchase receipts for verification of compliance.

Noncompliance Area: The facility lacked a sufficient amount of toys to handle and manipulate in the young toddler classroom.

Correction Required: Play equipment and materials appropriate to the developmental needs, individual interests and ages of the children shall be provided in sufficient amount and variety to preclude long waits for use. Play equipment and materials shall include toys to handle and manipulate and art materials for tactile development. Obtain additional of toys to handle and manipulate in a sufficient amount to preclude long waits. Submit written documentation of purchase receipts for verification of compliance.

Noncompliance Area: The refrigerator temperature read 50 degrees.

Correction Required: A facility shall have an operable, clean refrigerator used to store potentially hazardous foods. The refrigerator shall be capable of maintaining food at 45 F or below. An operating thermometer shall be placed in the refrigerator. Lower the refrigerator temperature to at least 45 degrees and ensure that it remains at the required temperature as specified.

Noncompliance Area: Three staff persons were observed caring for a group of preschool age children and one of the staff persons was not able to name specific children she was responsible for supervising. (staff person #2)

Correction Required: Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. Train all facility staff person in proper supervision of children, including procedures for assigning staff the responsibility for supervision of specific children as well as for staff persons knowing the names of the children in their assigned group. Submit written verification of the completed training for all facility staff persons. Submit written documentation of the facilitys procedures for assigning supervision responsibility.

Noncompliance Area: Children were observed being cared for in the infant classroom and one child's emergency contact information was not present in the classroom.

Correction Required: When children are in the facility, emergency contact information shall be present in a child care space for children receiving care in the space. Ensure that emergency contact information is present in all child care spaces in which children are receiving care.

Noncompliance Area: One child's record contained a health report completed more than 60 days (6 months) following the first day of attendance at the facility. (#1)

Correction Required: The operator shall require the parent of an enrolled child, including a child, a foster child and a relative of an operator or a facility person, to provide an initial health report no later than 60 days following the first day of attendance at the facility. Revise written policies and procedures to ensure that childrens initial health reports are completed and provided as specified in this regulations. Submit a written copy of the revised policies and procedures.

Noncompliance Area: One child's record did not contain a current up-to-date health report that had been signed by a physician, physician's assistant or a CRNP. (#3)

Correction Required: The operator shall require the parent to provide an updated health report at least every 12 months for an older toddler or preschool child.A health report must be written and signed by a physician, physician's assistant or a CRNP. The signature must include the individual's professional title. Obtain and submit a current up-to-date health report that contains the signature and professional title of the physician, physician's assistant or a CRNP. Ensure that all childrens records include a properly signed and dated health report as specified in this regulation.

Noncompliance Area: Children were observed eating snack in two classrooms (preschool and school-age) and staff did not ensure that the children's hands were washed before eating.

Correction Required: A staff person shall ensure that a child`s hands are washed before meals and snacks, after toileting and after being diapered. Train all staff persons in proper hand washing requirements and procedures. Submit written verification of the completed training for all facility staff.
