The staff member assigned to space #6 was inside the classroom space #6 talking to another staff member and was unable to see the children that were in the hallway. While the seven (7) children were in the hallway, one child walked down the hallway and entered the bathroom alone.

According to the posted menu, lunch was noted as breaded chicken patty, peas, hashbrowns, fruit cocktail, whole grain bread, and milk however the children were served grilled chicken patty, peas, hashbrowns, fruit cocktail, captain's wafer crackers, and milk.
