The room with 2 year olds and older was not within staff/child ratio. 1 year old room did not have a personnel present when worker arrived. Supervision was not met. |

Plan: The plan of correction for this incident will be to make sure that my staff do not leave their assigned rooms. We will not give breaks to other staff until we are positive that no one else needs to leave their classroom. All staff will complete an in-person training over procedures. |
