Noncompliance Area: During the Renewal Inspection on 10/15/2019, there was play equipment with cracks and sharp edges exposed seen on the larger outdoor play area.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Noncompliance Area: During the Renewal Inspection on 10/15/2019, the agreements for 3 of the 5 child files that were reviewed were not signed by the operator.

Correction Required: An agreement shall be signed by the operator and the parent.

Noncompliance Area: During the Renewal Inspection on 10/15/2019, Child #1 had an updated health report on file. However, the only health report on file Child #1 was dated 9/25/2019. Child #1 has attended the facility since 9/11/2018.

Correction Required: The operator shall require the parent of an enrolled child, including a child, a foster child and a relative of an operator or a facility person, to provide an initial health report no later than 60 days following the first day of attendance at the facility.

Noncompliance Area: During the Renewal Inspection on 10/15/2019, the diaper changing pad that was being used for diapering in the infant/young toddler room was ripped with foam exposed; thus it could not be sanitized properly.

Correction Required: Diaper changing surfaces shall be cleaned after each use by wiping the surface with a sanitizing solution or by changing a pad or other surface covering.

Noncompliance Area: During the Renewal Inspection on 10/15/2019, Staff #1, Staff #2 and Staff #3 all had up to date health assessments, but their initial health assessments were conducted several months after they started working at the facility.

Correction Required: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem.

Noncompliance Area: During the Renewal Inspection on 10/15/2019, peeling paint was seen in the preschool classroom.

Correction Required: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility.

Noncompliance Area: Staff #1 was hired on 3/17/2018, but did not have an initial health assessment and tuberculosis screening by the Mantoux method for initial employment on file on 10/5/2018.

Correction Required: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem.An adult health assessment must include tuberculosis screening by the Mantoux method at initial employment. Subsequent tuberculosis screening is not required unless directed by a physician, physician's assistant, CRNP, the Department of Health or a local health department.

Noncompliance Area: Staff #2 was hired on 4/25/2018, but did not complete the mandated reporter training until 8/10/2018.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). All staff must complete the mandated reporter within 90 days of hire as required by the CPSL.

Noncompliance Area: A container of glass cleaner was seen in an unlocked cabinet underneath the sink in the cafeteria.

Correction Required: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children.

Noncompliance Area: In the outdoor playground area a tricycle with cracked plastic and two play tables with cracked plastic were seen. The gate for the toddler play area was also separated from the fence.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Noncompliance Area: The facility fire drill log is currently up to date, but there was a lapse when a fire drill was held on 5/5/2018 and not again until 7/18/2018.

Correction Required: A fire drill shall be held at least every 60 days. Facility persons and children in attendance shall participate in the fire drill. Facility persons and children shall exit the building, weather permitting.

Noncompliance Area: During a complaint inspection on 6/5/2018, staff had all of the children who were in care at the time outside in the parking lot area in front of the facility close to where cars enter and leave the parking lot. The infants and toddlers were in strollers on the walkway, but the preschool aged children were observed playing in the parking lot and the side of the building. There is not any fencing or other natural barriers to restrict the children from the unsafe areas where cars enter and leave the parking lot.

Correction Required: Conditions at the facility may not pose a threat to the health or safety of the children. If unsafe areas or conditions are in or near an outdoor play space, fencing or natural barriers are required to restrict children from those unsafe areas or conditions.

Noncompliance Area: During a complaint follow-up inspection on the morning of 6/12/2018, Staff #1 was observed working in a classroom by herself with a mixed age group of 12 preschool and school age children.

Correction Required: When children are grouped in mixed age levels, the age of the youngest child in the group determines the staff:child ratio and maximum group size in accordance with the requirements in 3270.51 (relating to similar age level). The required staff:child ratios must be maintained at all times.

Noncompliance Area: During a complaint investigation on 6/5/2018, it was determined through staff interviews that the grass in the outdoor play area was knee high on staff four days earlier. This made length of grass too high for children to play safely in.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

Noncompliance Area: Two electrical outlets in the infant/young toddler room were observed to be missing receptacle covers.

Correction Required: Protective receptacle covers shall be placed in electrical outlets accessible to children 5 years of age or younger.
