Noncompliance Area: Based on interviews it was determined that Staff #1 has used threatening language towards children in care.

Correction Required: A facility person may not single out the child for ridicule, threaten harm to the child or the child's family and may not specifically aim to degrade the child or the child's family.

Noncompliance Area: Based upon interviews with staff members, it has been determined that Staff #1 has used harsh, abusive language with children in care. It was also determined, based upon interviews, that Staff #1 used threatening language towards children in care

Correction Required: A facility person may not use harsh, demeaning or abusive language in the presence of children.

Noncompliance Area: The following were observed n areas accessible to children in care: A metal baseboard heater located in the school-age hallway was observed to have sharp edges, Also in the hallway, An approximately 2'x4'x2' plastic storage bin was observed to have cracked, jagged edges and, in the older preschool room, an approximately 12"x18" plastic bin was observed to have cracked, jagged edges. In the outside play space, an approximately 3' x 5' wooden playset was observed to have areas of splintering wood that could cause puncture injuries.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Noncompliance Area: In the Pre-K room, exposed bolts were observed underneath the tabletop where each of the table legs were attached. In the Older Preschool room, an exposed nail was observed on the top of a wooden storage shelf. Noted areas of concern were observed in areas accessible to children in care and could cause puncture and/or laceration injuries.

Correction Required: Furniture must be durable, safe, easily cleaned and appropriate for the child's size, age and special needs.

Noncompliance Area: A written plan identifying the means of transporting a child to emergency care was not posted in the Pre-K room.

Correction Required: A written plan identifying the means of transporting a child to emergency care and staffing provisions in the event of an emergency shall be displayed conspicuously in every child care space and shall accompany a staff person who leaves on an excursion with children.

Noncompliance Area: The file for Child #1did not include documnetation of a flu vaccine in the past 12 months.

Correction Required: A health report shall include a review of the child's immunized status according to recommendations of the ACIP.

Noncompliance Area: Although the files for Staff #2 and #3 contained current health assessments; the previous health assessments for each staff member were completed more than 24 months prior to the current,

Correction Required: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem.

Noncompliance Area: The sheets on the mattresses of three cribs were determined to be ill-fitting as they were too large presenting a threat of the bunching of fabric and, in turn, a suffocation hazard.

Correction Required: Conditions at the facility may not pose a threat to the health or safety of the children.

Noncompliance Area: The files for Staff #1 and #4 contained signed/dated disclosure statements; however, said documents were dated after each staff member's first date in child care positions. The file for Staff #5 contained documentation of the completion of the PA State Police clearance; however, the filed form is not on a DHS OCDEL accepted form.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). LACKING REQUIRED HIRING DOCUMENTS: Facility Person # 5 may not work in a child care position at the facility. A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Facility Person #5 may not work in a child care position at the facility.

Noncompliance Area: In the School Age 1 room, in an area accessible to children under the age of six, two exposed electrical outlets were observed.

Correction Required: Protective receptacle covers shall be placed in electrical outlets accessible to children 5 years of age or younger.

Noncompliance Area: A Swiffer mop with an attachment containing a disinfectant solution was observed in an area accessible to children in care in the Pre-K room.

Correction Required: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children.

Noncompliance Area: In the Pre-K room, a container of Chlorox cleaner was observed being stored atop a cabinet above food items to be eaten by children in care. As there was not a protective barrier between said items, the potential was present for the cleaning liquid to leak onto the food items.

Correction Required: Cleaning materials and other toxic materials shall be stored in an original labeled container or in a container that specifies the content. Toxics shall be stored away from food, food preparation areas and child care spaces.

Noncompliance Area: A first-aid kit was not present in the Pre-K room.

Correction Required: A first-aid kit shall be in a child care space.

Noncompliance Area: Observed on the wall in the Older Pre-school room in an area accessible to children in care was an approximately 12"x30" heating vent with six exposed bolts. In the School Age 2 room, four bolts were observed to be protruding from the wall. In the Gym, sharp metal brackets were observed in an accessible space behind the bleacher. Each noted area of concern poses a threat to cause laceration and/or puncture injuries.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

Noncompliance Area: In the boys restroom located in the hallway, two toilets were each observed to have exposed, rusted bolts protruding from the base that attaches to the floor. Exposed bolts could cause laceration and/or puncture injuries.

Correction Required: Toilet areas and fixtures shall be cleaned daily and be in good repair.
