|Findings: The Surveyor reviewed five emergency cards and found the following; a) Child 1 was missing a date of enrollment and at least two emergency contacts. b) Child 2 and Child 3 were missing a date of enrollment. c) Child 4 was missing a date of enrollment, at least two complete emergency contacts and a health care provider name and number.|
|Findings: The Surveyor observed 11 children present but only 8 were signed in on the day of the inspection.|
|Findings: The provider only had 3 cots and 2 cribs in the home for children to nap on but there were 8 children and 3 infants present at the time of the inspection.|

|Findings: The Surveyor observed an infant in a car seat drinking a bottle of milk.|

|Findings: The Surveyor found that Staff 1 failed to apply for a fingerprint clearance card within seven working days of employment. Staff 1 start date: 2.6.17. An application had not been submitted as of 4.24.17.|
