Children were not adequately supervised at all times. At 11:10 am two staff members were observed outdoors with 14 children ages 4 - 5 years of age. Both staff members were observed sitting at the picnic table on their cell phones rather than adequately supervising children.

Surfacing did not extend six (6) feet beyond the external limits of the equipment or 3 feet for equipment used only by children less than 2 years of age. The new ninja obstacle course was placed 7 feet 6 inches to the climbing structure which does not meet the required 12 feet of fall zones between the two structures. Both pieces of equipment measured over 30 inches in critical height. Also, a black containment boarder was located in the fall zone for the ninja obstacle course.

Outdoor play equipment was not age and developmentally appropriate. A ninja obstacle course was added to the outdoor playground. This piece of equipment is rated for children 5 years of age and older. Children as young as 2 years of age were observed using the new piece of equipment.
