The Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy was not reviewed with new staff prior to providing care with children and/or a signed acknowledgement with all the required information was not maintained in the staff person's file. One new staff person, hire date 07/17/2022, did not have proof of the Prevention of Shaken Baby Syndrome and Abuse Head Trauma policy having been reviewed with her. The staff signed the review/receipt of the document during the visit.

All beds,cots, or mats with individual linen were not provided for each child. In Space 4 where the children ages 3 to 7 years old were located, there were seven children laying on cots with no linens. The children were asked to get up and the clean linens were placed onto the cots.

Each child was not attended to in a nurturing and appropriate manner, or in keeping with the child's developmental needs. Upon arrival it was observed that the linens were not on the cots. While the staff was attempting to get the sheets onto the cots, she spoke directives to the children in loud harsh tones, saying "Get up" and "Move".

On or before the first day of work, all staff, including the director and individuals who volunteer more than once per week did not provide results indicating that they were free of active TB and/or TB test or screening was older than 12 months. One new staff hired 07/17/2022, the cook/floater, did not have proof of being free of TB. A test was observed having been administered but not read. The Administrator reported that she was to have it read later today.

Prior to employment a Criminal Background Check was not completed. One new staff, hired 07/17/2022, Angela Pearson, had submitted the paperwork and fingerprints for the Criminal Background Check but it had not been completed prior to her employment. She was verified as having submitted the required information, in the Regulatory database, but that there are no results as of yet.

The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. The Administrator was unable to located the certificate for the training. At the 02/23/22 visit she was instructed that if she could not find the certificate she would need to retake the training. The training has not yet been completed therefore not meeting the requirement of completion within 90 days of employment.

Menus for all meals and snacks were not current or posted where easily seen by parents and cook. A current menu was posted in the kitchen and the classroom where the one and two year old children were located but was not posted in the classroom where the preschool children were located and therefore not easily seen by the parents.

Food substitution was not of comparable food value or recorded on the menu prior to the meal or snack being served. The cook had listed one food substitution for the hot dogs, listing meat balls but the second food substitution for the fruit was not listed as they were served applesauce. Although the one substitution of the two required documented substitutions was listed in the kitchen; it was not listed in the classrooms where the children eat.

NP had a TB test completed but it was over one year old.. No appointment has been scheduled. This violation was cited at the 02/23/22 visit and has not yet been corrected. No appointment has been scheduled. All violations must be corrected immediately (repeat violation).

New staff, who had contact with children, did not receive at least 16 hrs. orientation within first 6 weeks. NP had all items in the topic areas required in the 1st two weeks completed but had not yet had any training on Medication Administration which is required in weeks 3-6 of the orientation (repeat violation).

Application for enrollment did not include all required information. The missing items on the applications from the six children's files was highlighted at the 02/23/22 visit and it had not been completed/corrected on KM, AC, JF, JB and KG files. Based on a review of these files the application still did not include all required information (repeat violation).
