We encourage families to contact the daycare provider directly with any questions or concerns, as the provider may have already addressed some or all issues. Childrens records shall contain the following information:Application form which includes childs name, date of birth and address, name of parent or guardian, telephone numbers (home and business), work hours of parents or guardians, and date of enrollment in facility.

However, it is recommended that care be taken to maintain a constant depth as displacement may still occur.
