|Findings: a) Surveyor reviewed two personnel files. The file for Staff 1 had a fingerprint clearance card that expired on 2/16/18. According to the DPS website, they have not received a renewal application.|
|Findings: Surveyor 2 reviewed one staff file and found that Staff #1 (start date 08/15/07) did not have documentation of submitting the Central Registry Background check. The deficiency was acknowledged by the Provider on the telephone on 03/06/18.|
|Findings: Surveyor 2 requested to view the staff attendance and was told by the Provider that she does not sign in or out everyday. According to Staff #1, she does not work on a regular basis and did not sign in for the day. Staff #2 also stated she does not sign in or out everyday.|
|Findings: Surveyor 1 observed the parent postings and found that the hours of operation, weekly activity schedule, and screen time were not posted.|
|Findings: Surveyor 2 reviewed thirteen children's emergency cards and found the following: a) Child #1 and Child #2 did not complete the back side of the card that contains medical information and the parent signature. b) Child #3, Child #4, Child #5, Child #9, and Child #11 were missing the name of the person to call first in case of an injury or illness and and did not complete the back side of the card that contains medical information and the parent signature. c) Child #7 and Child #8 were missing the physician's name and telephone number, the name of the person to call first in case of an injury or illness, and did not complete the back side of the card that contains medical information and the parent signature. d) Child #10 was missing the physician's name and telephone number and the name of the person to call first in case of an injury or illness. e) Child #12 was missing date of enrollment, mother's address, the physician's name and telephone number, the name of the person to call first in case of an injury or illness, and did not complete the back side of the card that contains medical information and the parent signature. f) Child #13 did not have an emergency card and was present at the home.|
|Findings: Surveyor 2 reviewed thirteen children's files and found that Child #3, Child #4, Child #5, Child #6, Child #7, and Child #8 did not have a copy of the immunization record.|
|Findings: Surveyor 2 reviewed the children's February attendance records and found that Child #9 and Child #10 were not signed out on the 9th. On February 20 (inspection date), the children were signed out for later that day at 9:00 pm. Child #1 and Child #2 were signed in and out with initials for most of the month. Child #3, Child #4, and Child #5 who were present at the home did not have an attendance record. Child #6, Child #7, Child #8 were present and not signed in on the 20th (day of the inspection).|
