|Findings: The surveyor reviewed 9 enrolled children's Emergency, Information, and Immunization Record cards and observed the following: Child #1 lacked the name of the individual to be contacted in case of injury or sudden illness of the child. Child #2 lacked a date of enrollment.|
|Findings: The surveyor reviewed 9 enrolled children's immunization records and observed the following: Child #2...the name of the child was not on the immunization record for correct identification. Child #3...did not have an immunization record.|
|Findings: The surveyor reviewed 3 staff files and observed the following: Staff #1 did not have a copy of her fingerprint clearance card, new staff training documentation or reference check documentation in her file. Staff #2 did not have a copy of her fingerprint clearance card in her file, but had DPS verification of her valid card in her file.|
|Findings: The surveyor observed the following: One's/Two's room...an outlet located low and accessible on a wall in the room was not inset in the wall; it did not have a cover plate but was covered with duct tape.|
|Findings: The surveyor observed the following: Staff #2 was standing at a table, with her back to enrolled children, making playdough while children were eating the lunch provided by the facility. Staff #2 confirmed that the children's food had been served to them by a staff member and that children had not served themselves. Staff #2 was not seated at the table with the enrolled children, eating the staff member's own lunch.|
|Findings: The surveyor observed the following: Outdoor activity area...there were large ants on an ant hill located by the climbing equipment in the preschool playground area.|
|Findings: The surveyor observed the following: Kitchen...a package of powdered sugar was stuck to a cabinet shelf by a sticky substance in the cabinet; the cabinet shelf housing food was not clean.|
