Findings: Upon arrival there was a new staff person working. The director reports that the aide started on 2/1/2019 and that the director faxed the documentation to OCC. The documentation wasn't received and OCC didn't know of this new staff person. Lic. Spec. contacted the office while on site today and the OCC region 10 fax log was examined by OCC staff at region 10 and there was no entries that a fax was received from Younger Years or the director from January to the present. A completed original notarized release was received on site for the new staff person. However, the release form completed was an old form from 8/07. Director was asked to have a new release form completed for the new staff person and to immediately mail the completed original notarized release to OCC. The completed "1203 Add form" was received on site today. The OCC has now been notified. Please in the future contact the OCC to verify information was received. |
