Findings: Surveyor 2 reviewed 10 children's emergency cards that are kept by the staff in their classrooms for emergency's and observed that all 10 children did not have a copy of their immunization record attached to their card. The Director had the immunization records in a file cabinet in the preschool office in the children's individual files.
