Findings: Based on staff interviews, Staff 2 and Staff 3 reported that the children eat lunch in the school cafeteria. Upon review of facility documentation, Surveyor 1 determined that the cafeteria has not previously been licensed and the facility staff has not requested a modification to their license to include the cafeteria in their licensed space.
