One incident report dated December 5, 2022 for a child in space #1 did not include the time the parent was notified of the injury and by whom, steps taken to prevent reoccurrence, and the signature of the person completing the incident report.

Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months. One staff employed on 9/6/2022 did not have a medical statement completed until 9/20/2022.
