Noncompliance Area: Child # 4's rest equipment is ripped in multiple spots and not in good repair.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Noncompliance Area: Child # 1 DOB 5/9/17 has attended the facility since 10/11/17 and only has 1 child service report on file from 10/24/19. Child # 2 DOB 8/16//17 has attended the facility since 10/16/17 and only has 1 child service report on file from 10/28/19. Child # 3 DOB 7/17/15 has attended the facility since 3/20/17 and only has 1 child service report on file from 10/30/19.

Correction Required: The services to be provided to the family and the child, including the Department's approved form to provide information to the family about the child's growth and development in the context of the services being provided. The operator shall complete and update the form and provide a copy to the family in accordance with the updates regarding emergency contact information in 3270.124(f) (relating to emergency contact information).

Noncompliance Area: There is unlabeled, expired Parents choice diaper rash ointment in the caterpillar classroom changing table basket. The ointment expired 11/2019.

Correction Required: The label of a medication container shall identify the name of the medication and the name of the child for whom the medication is intended. Medication shall be administered to only the child whose name appears on the container. Medication shall be stored in accordance with the manufacturer's or health professional's instructions on the original label.

Noncompliance Area: The diapering cans in the Cocoon and Infant rooms are broken making them not hands free and lidded. The diaper can in the Caterpillar room is not a hands free can.

Correction Required: If disposable diapers are provided by a parent or by a facility, a soiled diaper shall be discarded by immediately placing the diaper into a plastic-lined, hands-free covered can.

Noncompliance Area: Facility person # 2 hired and working in child care since 8/29/19 did not have a health assessment at time of hire. Health assessment on file as of 10/18/19.

Correction Required: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem.

Noncompliance Area: Facility person # 1 hired and working in child care since 10/16/19, only has 1 reference on file.

Correction Required: A facility person's record shall include two written, nonfamily references from individuals attesting to the person's suitability to serve as a facility person

Noncompliance Area: Facility person # 2 hired and working in child care since 8/29/19 did not have proof of state police or child abuse clearance until 9/9/19. Facility person # 2 does not have mandated reporter training on file and has been employed in child care over 90 days. Facility person # 1 hired and working in child care since 10/16/19 does not have mandated reporter training on file and has been employed in child care over 90 days. Facility person # 1 hired and working in child care since 10/16/19 did not have proof of state police clearance until 12/17/19 and proof of child abuse clearance until 12/20/19. Facility person # 3 hired and working in child care since 5/23/19 did not have FBI clearance until 5/24/19.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL.

Noncompliance Area: There are no scissors in the Cocoon's first aid kit. There are no scissors or tweezers in the School Age room first aid kit.

Correction Required: A first-aid kit must contain the following: soap, an assortment of adhesive bandages, sterile gauze pads, tweezers, tape, scissors and disposable, nonporous gloves.

Noncompliance Area: Facility person # 1 , who is an aide was alone with 4 infants.

Correction Required: An aide or a combination of aides shall be supervised at all times by a staff person qualified at minimum as an assistant group supervisor.

Noncompliance Area: On 9/5/19 a complaint investigation was conducted. Facility staff person # 1 admitted to popping child # 1 on the leg to direct the child to the bathroom stall when child # 1 asked to use the bathroom.

Correction Required: Daily activities shall promote the development of skills, social competence and self-esteem. Daily experiences shall recognize the child as an individual and give some choice of activities that respect personal privacy, lifestyle and cultural background.

Noncompliance Area: On 9/5/19 a complaint investigation was conducted. Facility staff person # 1 admitted to popping child # 1 on the leg to direct the child to the bathroom stall.

Correction Required: A facility person may not use any form of physical punishment, including spanking a child. Facility person # 1 will complete a minimum of 2 hours in person PQAS approved training on redirection of children. The training will be approved by the Northeast Regional Office before it is scheduled. The training hours will not count towards facility person #1's required yearly child care training hours.

Noncompliance Area: On 9/5/19 an unannounced monitoring inspection was conducted. A review of the file for child # 1 was done. Upon reviewing child #1's file, child # 1 has been attending the facility for more than 60 days, since 2/11/19 and has no health assessment on file..

Correction Required: The operator shall require the parent of an enrolled child, including a child, a foster child and a relative of an operator or a facility person, to provide an initial health report no later than 60 days following the first day of attendance at the facility.

Noncompliance Area: On 9/5/19 an unannounced monitoring inspection was conducted. A review of the file for child # 1 was done. Upon reviewing child #1's file, child # 1 has been attending the facility since 2/11/19 and has not had the fee agreement or emergency contact forms updated in over 6 months since 2/8/19.

Correction Required: A parent is required to review and update the emergency contact information and the financial agreement at least once in a 6-month period or as soon as there is a change in the information.

Noncompliance Area: The handbook and information that is given to parents at enrollment does not include a general daily schedule, hours for which care is provided and supervision policies.

Correction Required: At the time of enrollment, a parent shall receive in writing the facility's general daily schedule, hours for which care is provided, fees, responsibilities for meals, clothing, health policies, supervision policies, night care policies, dismissal policies, transportation and pick-up arrangements.

Noncompliance Area: In the multipurpose room in the back left corner where the television is hanging, inspector observed cords from the television loosely hanging. Cords are accessible to the children.

Correction Required: Conditions at the facility may not pose a threat to the health or safety of the children. Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.
