Findings: Based on the review of facility documentation and interview with the staff , it was determined that the facility staff were not taking a copy of the Emergency, Information and Immunization Record cards of the enrolled children on the field trip to the swimming pool. The Director stated that since the swimming pool was located at the facility, staff were not taking the Emergency, Information and Immunization Record cards with them.

Findings: Based on Surveyor review of 7 staff files, and information provided by staff, Staff #2 and Staff #3 were lacking documentation of a negative Mantoux skin test or other tuberculosis screening test, administered within 12 months before the starting date of employment. Staff #2's start date was 03/15/19, and the TB test on file was dated 05/24/2019. Staff #3's start date was 05/25/2019, and the TB test on file was dated 06/12/2019.
