A safe indoor and outdoor environment was not provided for the children. In Space 1, approximately three nails were observed protruding from wooden steps to a loft which present a potential safety issue. At the corners of the playgrounds, as well as along the fence dividing the playgrounds, where the metal fencing meets, and at all gate junctions where the gate doors either connect with each other, or with the fencing itself, greater than ten (10) bolts, which secure the fencing were protruding inward from the gate posts, each measuring between one half inch and one inch in length beyond the nuts, presenting potential safety hazards. Two border tie tacks on the playground used for infants and toddlers were observed to be raised and present a potential entanglement hazard. Two border ties on this playground were also observed to be rusting. The rim along the top step of the stationary structure on this playground was observed to be cracked and separated. Rust was observed along the top of the fence rails bordering the playground that are not coated (those initially silver in color, as opposed to the black section of fencing, which is unaffected). To the left of the gate entrance to the playground used for children of Pre-K and School Ages, the space between the brick wall and the gate post measures 5 inches, presenting a potential entrapment hazard. A plastic door mounted to the bottom of the stationary play structure on this playground was observed to be cracked and broken at the top and bottom hinges. | |||
